  • An assessment is initiated within 48 hours of referral
  • Services providing 3 to 12 hours of contact per week, per individualReducing psychiatric symptoms
  • Educational intervention aimed at understanding the course of psychiatric illness and prevention and management of the acute phases of the condition
  • Identifying individual's behavioral assets and strengths
  • Providing services according to individualized needs:
    • Referrals are accepted 24 hours a day, 7 days a week

  • Appropriate clinical documentation in the care record:
    • Maintain progress notes
    • Individual/Family assessments

    • Discharge planning
  • Maintaining small caseloads (max 5 cases per counselor)
